Disc Miniscus
Today’s “Case of the week” is about a 45-year-old patient who tore her anterior cruciate ligament (ACL) in 2017. At that time, the injury was treated surgically and the anterior cruciate ligament with the semitendinosus tendon was reconstructed arthroscopically. However, the knee kept giving me problems and there was recurrent swelling in the joint.
Diagnosis and treatment
During a checkup in April 2022, a painful extensor deficit was clinically noted and a cyclops syndrome was diagnosed by MRI scan.
Cyclops syndrome is a form of arthrofibrosis and presents as spherical scarring at the attachment of a cruciate ligament graft. A typical clinical sign is a persistent and painful extension deficit in the knee joint due to the mechanical blockage. In another minimally invasive procedure, the cyclops tumor is removed, leaving the patient pain-free and fully mobile.
Behandlung
Die Behandlung des Scheibenmeniskus ist abhängig vom individuellen Leidensdruck. Ein unauffälliger Scheibenmeniskus, der bei Patient:innen keine Beschwerden verursacht, bedarf keiner operativen Versorgung. Liegt hingegen ein Einriss vor, der Schmerzen auslöst, erfolgt die Behandlung durch eine arthroskopische Teilmeniskektomie, auch Meniskusremodeling genannt. Dabei wird das Volumen des Meniskus reduziert und er wird in die physiologische Sichelmondform gebracht. Dadurch wird die für die Funktion und Stabilität essentielle Ringstruktur wiederhergestellt.
